GST invoice format for an imaging and radiology centre

X-ray, ultrasound, CT and MRI scans done to diagnose a patient's illness are health care services of a clinical establishment and exempt from GST under SAC 999316. An imaging centre's bill also needs the referring doctor, scan type with body part and contrast use, the radiologist reporting it, and for obstetric ultrasound, the PCPNDT Form F reference. Equipment the centre buys, such as X-ray and ultrasound machines, is now taxed at 5%, which the exempt centre cannot recover as credit.

Extra fields a imaging and radiology centre invoice needs

Scan type and body part
Show the modality and region, such as 'MRI lumbar spine, plain'. This prevents disputes over which scan was done and supports insurance claims that pay by procedure.
Contrast used
Contrast studies cost more. Recording 'with contrast' and the agent used explains the higher charge and supports the clinical record if the patient reacts later.
Referring doctor
Scans are done on a doctor's request. Naming the doctor links the scan to the prescription and helps the patient's claim.
Reporting radiologist
The radiologist who signs the report should be named. It shows the scan was interpreted by a qualified doctor and supports the exemption.
PCPNDT Form F reference
For any pregnancy ultrasound, the law requires Form F to be filled. Linking the bill to the Form F serial number keeps the two records matched for inspections.
Film or CD and report delivery
Patients may need films, a CD or an online report link. Showing what was handed over and when avoids repeat requests.

GST rules that matter for a imaging and radiology centre

Diagnostic imaging is exempt health care

The exemption covers health care services by a clinical establishment, and the definition of clinical establishment includes centres offering diagnostic or investigative services. An imaging centre doing scans for patients sent by doctors supplies exempt health care. This includes the scan, the contrast agent used, the films and the radiologist's report, since all are part of one service. A centre with only exempt income need not register. Tele-radiology reporting by a radiologist for a patient is also health care, even when the radiologist is in another city.

Equipment and consumables are a cost at 5%

Apparatus based on X-rays or radiation for medical use, including CT, X-ray tubes, generators and examination tables (9022), and medical instruments including ultrasound units (9018), moved from 12% to 5% from 22 September 2025. Because scan income is exempt, the centre cannot take credit for this tax. The cut directly reduces capital cost. Record equipment cost inclusive of GST in your fixed asset register, since depreciation for income tax is claimed on the full cost when credit is not taken.

Annual maintenance contracts on scanners

Scanner AMCs and CMC contracts with manufacturers are expensive and are taxable services. That tax cannot be credited and adds to running costs. Compare AMC quotes on a tax-inclusive basis. If the AMC includes spare parts, the vendor decides whether it is a composite service or a mixed supply; the rate on its invoice is its responsibility, but you should check it is consistent from year to year.

Reading services sold to other centres

Some centres earn income by reporting scans for other hospitals or diagnostic chains. When the reporting is for a patient's diagnosis, it is health care. When a centre rents out scanner time to another operator without providing any reporting or patient care, the income is a rental or business service and taxable. Keep contracts clear about which is being sold.

PCPNDT and other registrations on bills

Ultrasound centres must be registered under the PCPNDT Act and display the registration. Print the registration number on bills along with the AERB licence references for X-ray and CT if you wish. These are not GST requirements, but inspectors and insurers look for them and a bill showing them is less likely to be questioned.

Corporate health check contracts

Employers and insurers buy bulk imaging as part of health check-ups. These are still exempt when done for the employees' diagnosis or screening. Show the employer's purchase order and the employee list on the invoice. Since no tax is charged, there is no e-invoice requirement for these supplies.

Common billing mistakes

  • Charging GST on contrast or film as separate taxable goods within an exempt scan bill.
  • Not linking obstetric ultrasound bills to the PCPNDT Form F record.
  • Treating rental of scanner time to another operator as exempt health care.
  • Claiming input tax credit on new machines against exempt scan income.
  • Omitting the reporting radiologist's name, which insurers and patients may need.

Imaging and radiology centre GST invoice: FAQ

Is GST charged on MRI and CT scans?

No. Scans done to diagnose a patient's condition are exempt health care services of a clinical establishment.

Is GST charged on contrast used in a scan?

Not separately. Contrast is part of the scan service, which is exempt as a whole.

What is the GST rate on X-ray and ultrasound machines?

X-ray apparatus (9022) and medical instruments (9018) are taxed at 5% from 22 September 2025.

What SAC code should an imaging centre use?

999316, which covers medical laboratory and diagnostic imaging services.

Do imaging centres need GST registration?

Not if all income is from exempt scans. Taxable income, such as renting scanner time, can trigger registration once aggregate turnover crosses the threshold.
